How Our In-Home Consultations Work

What to Expect When You Meet Our Experienced Team

Initial Assessment

 The Care Manager will ask a variety of questions to identify areas where your loved one may need assistance, while also recognizing the aspects of daily life they are managing well on their own.

Review of ADLs

 Next, the Care Manager will inquire about common Activities of Daily Living, such as bathing, dressing, cooking, and transferring in and out of bed or chairs. Are these tasks still manageable, or do they require some assistance?

Recognize Areas for Assistance

 After assessing the situation, the Care Manager will discuss care options that could benefit the family, addressing any gaps or needs identified.


Home Safety Assessment

 A home tour will help identify potential safety hazards, such as trip risks, the absence of grab bars in wet areas, improper use of extension cords or space heaters, or any other environmental concerns.

Social Well-being Evaluation

 Next, the Care Manager will assess any physical or emotional risks related to isolation and loneliness.

Care Management Plan

 The Care Manager will then review any health concerns, medication management, and other issues that should be included in the care coordination plan.

Care Plan Recommendations

 We will go over the results with you and provide recommendations. Together, we can discuss the appropriate hours and types of care that will best support your family.
